Sometimes two or more techniques are used in combination to get the best result. Among the transplant techniques, techniques such as punch graft, mini graft, micrograft, slit graft and strip graft are generally used in patients requiring smaller procedures. Techniques such as flaps, tissue expansion and scalp reduction should be applied to patients who require more dramatic changes. Today, hair transplantation with the Fue method is widely performed. Especially mini grafts are a good option for these patients to fill the areas with thinning hair. The back of the head, where the hair strands are denser, should be used as the donor area. Flaps and tissue expansion procedures can be used in patients with excessive hair loss. FEATURES OF HAIR TRANSPLANTATION SURGERY
H air transplantation is the transfer of the scalp in small pieces from a donor area to a balding or sparse area. Grafts can be of different sizes and shapes. Round shaped punch grafts contain approximately 10-15 strands of hair. Smaller miniggrafts, on the other hand, contain 2-4 strands of hair. Slit grafts placed in strips to shape the scalp contain approximately 10 strands of hair. Strip grafts are longer and thinner and contain approximately 30-40 hairs. Generally, surgical interventions may need to be repeated for a successful outcome, and it may take months for recovery between each session. In cases where all hair is transplanted, this may take up to two years. The color and quality of the hair also affect the outcome of the surgical procedure. A better result is obtained in thick and light-colored hair compared to dark-colored hair. The number of hair follicles taken in the first surgery varies according to the individual, but it is approximately 50. In mini grafts and micrografts, this number can increase to 700 for each cure. Before surgery, the hair in the donor area is shortened to place it more easily in the recipient area. In punch grafts, your doctor can use a special, tube-like instrument made of sharp carbon steel. With this, it takes the grafts from the donor area and places them usually on the hairline in the frontal area. In other types of grafts, they are taken from the hairy areas with the help of a scalpel and divided into small and thin pieces. Serum saline is injected periodically to the area where the grafts are taken to create more skin tension. In punch grafts, the donor area is closed with stitches. One stitch is sufficient for each punch graft area. In other graft types, a thin scar may be left. Stitches can be hidden by the hair around them. The grafts are placed at intervals of 1/8 of an inch for the first time to ensure healthy nutrition of the scalp. In the later stages, these intervals are filled. Your doctor takes the grafts with great care in order not to damage the donor area and places them in accordance with the natural growth direction of the hair while placing them. After the grafting process is completed, the scalp is cleaned and covered with clean gauze. A pressure bandage can be used for a day or two. Some physicians may discharge their patients without using bandages.
T issue expansion is a field in which plastic surgeons are experts and it is used for reconstructive purposes, especially in burns and cases with significant tissue loss in the scalp. With the use of this technique, very good results are obtained in a relatively short time. In this technique, a balloon-like substance called a tissue expander is placed under the skin in a hairy area adjacent to balding areas and inflated with a saline-containing serum at regular intervals. This causes the hairy area to grow and expand and new hair formation. Generally, approximately two months after the first surgery, there is sufficient tissue expansion and this balloon is removed with a second operation and the enlarged scalp is advanced to the balding area. Flap surgery: Flap surgery has been successfully applied for more than 20 years. With this method, hairless areas are covered with the scalp in a very short time. The expanse and placement of flaps are determined by the needs and wishes of the patient. A flap carries hair equivalent to 350 or more punch grafts. In this technique, a piece is removed from the balding area and the area with the scalp is moved towards the removed area. The resulting scar can be hidden by the surrounding hair. In recent years, very successful results have been obtained by combining flap surgery with tissue expansion or scalp reduction techniques. Thus, both more scalp is obtained and a more natural hairline is created. Reducing the scalp: This technique is sometimes called an advancement flap. Because the part removed from the scalp is closed by advancing the surrounding scalp. This technique is especially suitable for hair loss on the top and back. It is not useful in creating the frontal hairline. After local anesthesia is applied to the scalp, some of the balding skin is removed. The size and shape of the removed piece vary according to the patient's expectations. If a large piece needs to be removed, an inverted Y-shaped incision is preferred. The shape of the removed piece can be U-shaped, oval-shaped or other shapes. To cover the place of the removed piece, the surrounding hair tissue is loosened and closed with stitches by advancing. In the meantime, you may feel some tension and some pain at that point.
